application & evaluation steps
(1) SPONSORSHIP. Applicants for all three levels of membership require
the sponsorship of a NAMS-CMS or Retired Life Member. Please contact our office to find out about sponsorship.
(2) APPLICANT SCREENING. A review of the applicants experience and qualifications.
(3) ETHICS TEST.
(4) NAMS-CMS TEST. A test for members seeking CMS membership status.

Please submit your completed application form, your five reports (if required), a current professional resume or Curriculum Vitae, and your check for the application fee.
